I know exactly the situation he's describing... someone else made a map of all of Wales, and like me, built all the motorways for the convenience of whoever played it. There were several junctions were a single road just crosses over the dual carriageway, and the trucks were unable to turn and drive across the other carriageway and onto the single road, because the stupid arrows that dictate which paths are valid hadn't appeared properly.
Waypoints didn't help, especially for road vehicles as after I plotted the route around this bug for my trucks, I found they'd skipped past all the waypoints in their orders and were now attempting to head straight for their destination (and so getting stuck).
The best way to avoid this is to take the time to build proper slip roads and overpasses for your dual carriageways... however, even this only works until the local council starts building its own roads to connect to the dual carriageway, and then your BLOODY STUPID BUGGY UNFINISHED AI trucks decide they'll ignore all the waypoints and try and find their own way, ignoring the aforementioned carefully placed entrance sliproads.

The only way to have a motorway going straight into a city like that would be to build it on a bridge, so the only ways on or off it would be via the roads you built... otherwise it'll just get mucked up when the city expands.
So, it's the road building design AND the AI that need a kicking.
Mostly just CS who needs all the kicking, though.
