I'm having a problem in my game where my industries simply aren't generating goods beyond a small amount of 32 - 100. I also tried building new industries, coal specifically, and at first they do fine, varying between 500-1000, but eventually they drop to 32 and then disappear completely even though I'm constantly transporting goods for them.

What's the problem here?

you need to watch your station rating, it must at all times be above 66% to get optimal industry growth.

I have consistently maintained a rating above 66

This is geting ridiculous, I'm trying to play a new game and all the industries are disappearing on me. I can't play at all.

Any more details (newgrfs, version, attach the savegame)?

It happens in every game, I have Aviators Aircraft set, New Trees, North American Renewal Set, Heavy Equipment Set, Dutch Tram Set, N-A Vehicle Set, Industrial Stations Renewal, and Total Town Replacement Set 3.11. I have tried both OpenTDD 1.11 and 1.10

We need a savegame...

Oil wells in temperate climate do dry out. That's the only industry in the default industry set that will die even if they are well serviced.

Oil wells in the temperate climate will only decrease there production, never increase it. After a while the production will be randomly reduced to nearly zero and the industry will close. Try other industry types and post a new savegame if you can reproduce your problem with them.

The oilwells in that savegame disappeared literally minutes after the game started. That save game was made just a few minutes after the game start, and for each well only one truck managed to make it to the refinery before the wells disappeared. Also if you stick around for a while longer the farmland disappears too even if you are to transport goods from it. I had the same problems with the coal mines too.

You can always use cheats to keep the production values up...

This means I have to keep going through each and every industry I have on a regular basis.

as soon as I enabled smooth industry changes in your save it all seems to work normally...
