Yes, it was doing an ECS move as 5E49 and it SPAD'd - got caught by the catch points.

I suspect he just mis-read or didn't see the shunt signal properly...

Similar thing happened a while ago here - an Endeavour DMU had just finished an overhaul of the brakes, and the driver/shunter/guard left the brake off, and she rolled away, straight towards the main and went right through the catch points and ended up in the dirt. Was a little embarrassing to go past the maintenance centre and see the thing sitting in the dirt, especially considering it had just had work done too...

When the Pendolinos were first introduced there were some real problems at a couple of terminii in terms of the brakes.
I believe one Pendolino was scuffed and completely destroyed a set of buffers at Liverpool Lime Street, even though the driver had correctly reacted and had braked as per his training.
The driver is still probably out on his ear though. Its rare to see the catchpoints employed for a passenger train.
