Interesting. Patchman did not include the readme. Run grfcodec -h for use instructions.

In a folder somewhere. It doesn't matter where, although I suggest putting them all in the same folder.
Personally, I do not use either the TTD or OTTD folders; it's harder to blow things away that way.

That's what it's supposed to do. (Start, do its job, exit.) I strongly recomend running it from the command line. (I suppose that's not abolutely required, though.)
I suppose you could use the GRFWizard but I haven't the slightest clue how it works; you're on your own if you can't figure it out.
