Looking at the 2048x2048 scenarios that came with OTTD, I've found that they're all so amazingly detailed that it's just unplausible that they were made by hand (but not impossible, of course). So I'm wondering, if a SV0creator for OTTD (i.e. it supports OTTD and larger maps) exists, where is it?

I was not aware that /any/ 2048x2048 maps shipped with OTTD. the Africa scenario is only 1024x1024.

There is also the terraingensis or terragenesis or something. It does a great job of creating landscapes. Of course it doesn't do real-life terrain.
